1- Complete form "Enter Customer Information Below". When you get to account number, us your social security number here. Click Print/Submit.

2- Check Box for Bill Paying Agreement and Click the I Agree button, please make sure the white box has a check in it.

3- Print White Form, note the Routing Number and Custodial Account Number * for use at MyPay (This form can also be taken directly to your finance/payroll office if you do not start allotments online, to have the allotment completed there. If you start allotments ONLINE, please proceed, by closing window. Please note if you do not have a printer to write down the routing number and custodial account number for use at MYPAY.

4 – Please print this page as well for your instructions to follow at MYPAY. Click Continue at the bottom of the page and follow instructions for starting the allotment at MYPAY, remember to use control P, to print the last two pages at MYPAY which must be faxed to 800-838-0717 or 631-348-0804.

The Notification page may also be emailed to sales@harrisjewelry.com if you do not have a printer or access to a fax machine.
